To support our families, Chicago Public Schools has partnered with a licensed medical provider, named CIMPAR to offer school-required/recommended vaccines and physical health exams.
The vaccines and health exam will be available at your student’s school at no cost to your family at:

2. Select your desired option and click Add to Cart
3. Provide your information
4. Confirm the information that you inputted
5. Download and print/take a screenshot of your ticket

SCHOOL REQUIRED IMMUNIZATIONS
All 8 Required Immunizations including, Chicken Pox, Measles, Tetanus, and Meningitis ACWY
Vaccines are available to all eligible, school aged Chicagoans, regardless of insurance or immigration status.
Additional services available at appointments:
● School Recommended Immunizations, including COVID-19, Flu, Meningitis B and HPV
● School Physicals, limited availability
** Consent from a parent/legal guardian is required if ≤17yo.
