HEALTH REQUIREMENTS

 

Illinois requires these to be met by October 15. 

 

PRESCHOOL

Examination Requirements:

Physical Examination

Vision Examination

Immunization Requirements:

- DTP  or DTaP:  3 doses by 1 year of age. One additional booster dose by 2nd birthday

- Polio: 2 doses by 1 year of age. One additional dose by 2nd birthday. 3 doses for any child

   24 months of age or older appropriately spaced

- Measles, Mumps, and Rubella: 1 dose on or after the 1st birthday

- Haemophilus type b (Hib) Proof of immunization. Children 24-59 months of age without

   series shall show proof of 1 dose of Hib vaccine at 15 months or older

- Invasive Pneumococcal Disease (PCV) proof of immunization. Children 24-59 months of age without

   a primary series of PCV, shall show proof of receiving 1 dose of PCV after 24 months of age

- Hepatitis B, 3 doses appropriately spaced. Third dose must be administered on or after 6 months of age

- Varicella (Chickenpox Vaccine) 1 dose on or after 1st birthday

  

KINDERGARTEN

Examination Requirements:

Physical Examination

Vision Examination

Dental Examination (Due by April 15)

Immunization Requirements:

- Diphtheria, Pertussis, Tetanus - 4 or more doses of DTP/DTaP; with the last does qualifying

   as a booster if received on or after 4th birthday

- Polio - 4 doses with the last dose on or after the 4th birthday

- Measles, Mumps, Rubella - 2 doses–the first on or after the first birthday and the second

  dose no less than 4 weeks after the first dose

- Varicella (Chickenpox) - 2 doses–the first on or after the first birthday and the second

   dose no less than 4 weeks after the first dose

Immunization Recommendations:
- Hepatitis B - 3 doses appropriately spaced. Third dose must be administered on or

   after 6 months of age

 

SIXTH GRADE

   dose no less than 4 weeks after the first dose
