Kindergarten - 8th Grade
- To expedite the enrollment process, please print and sign all enrollment forms. You can drop off the forms at Door 14, upper grade building.
- If you do not have access to a printer, please stop by Door 14, upper grade building.
- If your child is currently enrolled in K-8th grade, you DO NOT need to re-enroll for next school year.
- If your child is currently enrolled in preschool and lives outside the attendance boundaries, you will have to apply through GOCPS if you want your child to attend kindergarten next school year.
You will need to provide the following document when registering the student:
- 2 proofs of Chicago residency such as electric or gas bill, voter's registration card, Illinois Driver's License, Illinois State ID and/or property tax bill
- Birth Certificate for the child(ren)
- Complete Health Examination Form (medical/physical exams)
Forms need to be filled out by your pediatrician. Students enrolling for the first time are required to submit proof of Illinois physical examination and current immunizations.
